The purpose of the NY Connects program is to provide consistent, comprehensive, locally based information and assistance, on long term services and supports (LTSS) to individuals, caregivers and families, to help them make informed choices and give assistance in accessing such services and supports, regardless of age or finances (public benefit programs or private pay).

Our goal is to help individuals remain in their homes as long as possible. NY Connects serves older individuals and individuals with disabilities of all ages.
